Purpose
Governance diagnostics exists to convert recurring symptom patterns into a precise structural diagnosis.
Rather than producing longer action lists at the point of failure, we identify the governance weakness upstream and define the smallest set of structural changes likely to shift outcomes.
What it does
-
Characterises recurring symptom patterns.
-
Describes behavioural adaptation.
-
Connects outputs to governance by testing intended governance against lived governance.
-
Assesses structural maturity as a state inferred from evidence about governance stability under pressure.
-
Produces a board-ready problem statement and a prioritised governance intervention roadmap.

Approach
Diagnostics is an evidence-based audit exercise rather than a document review alone.
We use structured sampling and triangulation (ISO 19011:2018) and field verification of decision rights, escalation routes, interface ownership and control prerequisites.
Typical outputs
-
Governance diagnostic report.
-
Structural–causal explanation of recurring patterns.
-
Structural maturity reading using qualitative low/high contrast patterns.
-
Prioritised governance interventions.

Typical timeframe
Most diagnostics complete in 2–6 weeks depending on portfolio size, access and urgency.
Why this works
Recurring patterns are structural evidence.
If governance is weak or fragile, behaviour and symptoms will reassert themselves regardless of messaging, training or local corrective action. Diagnostics locates the causal mechanism and directs effort where change is possible.
